REGISTRATION FORM INSTRUCTIONS
1. Sign up.
Make your plans now to attend by printing this page for your records, and then printing and completing the registration form.

2. Select your workshops by number.
You must indicate your first, second and third choices for each workshop session by placing the number corresponding to the workshop that you want in the blank.
3. Select your Monday luncheon choice
4. Return your registration by mail or fax.
Mail: Complete the registration form, include payment, and postmark by March 12th to avoid a late fee. Please make checks for the conference and the Institutes payable to PCANC.
Mail your payment(s) and your completed registration form to:

5. Watch your mail for confirmation.
A confirmation will be sent to you if you register by March 30th. If you have any questions, please call 1-877-443-2670 (toll free).
Conference Registration Form
A separate form is required for each participant. You may duplicate the form.
If you cannot attend for any reason, you must cancel your conference registration in writing. Phone cancellations cannot be accepted.
Cancellations postmarked prior to March 12, 2001 are subject to a $25.00 processing fee. After March 12, 2001, there will be no refunds or cancellations. Substitutes for registered attendees may be made without penalty. All refunds will be issued after the conference.
Your standard conference registration fee covers conference materials, credits, reception, and attendance at both plenary sessions, your choice of workshops, luncheons, and all coffee breaks. The fee for the NCPSAC Institutes is additional.
